SST Forms

  • BEFORE THE MEETING:
     
    1. Log into your Google Drive.
     
    2.  Click the form you need.
             a.  SST Academic Contract Form (1st meeting)
             b.  SST Academic Contract Form (Follow up meetings)
     
    3.  Click file, make a copy.

    4.  Title it.

    5.  It's ready for you to edit during the meeting.
     
    AFTER THE MEETING:
     
    1.  Print a copy for the student and parent to sign.

    2. Click Share.

    3.  Share it with all relevant parties (admin, counselors, teachers).

    4.  Log the meeting in AERIES under Interventions.  Put a copy of the shareable link in the description (when you click share on the document, there should be a shareable link you can copy and paste.)
